Dr. McAndrews Performs Both FUE & FUT
Follicular Unit Excision (FUE)
Follicular Unit Excision (FUE) surgically cuts tissue from the scalp using a sharp circular punch excision tool (usually 0.8–1.0 mm). The surgeon surgically excises thousands of grafts one at a time, working across a broad donor area of the back and sides of the scalp to avoid overharvesting in any one spot. This same size punch excision used for present day FUE hair transplants was used by a Japanese doctor, Dr. Okuda, back in 1939 to perform hair transplants. FUE leaves hundreds to thousands of small circular scars that are barely visible, even with short hair. With subsequent FUE surgeries, the circular scar tissue is not removed, so scar tissue accumulates with each successive surgery. The short-term cosmetic downside with FUE is that the scalp needs to be shaved completely in the donor regionin order to spread out these small circular scars, therefore it takes a few months of hair growth to hide the scars. Follicular Unit Transplantation (FUT)
Follicular Unit Transplantation (FUT), a form of strip excision, involves removing a thin strip of scalp from the back or sides of the head. This strip is precisely removed from the area of the best genetic quality hair. The tissue is then dissected under a stereoscopic microscope to carefully isolate intact follicular unit grafts. FUT is often preferred by patients who want the best quality and survival of the grafts, do not plan to shave their head, and/or want to preserve donor density. With subsequent surgeries, the prior scar is removed and replaced by a new fine linear scar. If performed by an experienced surgeon using a trichophytic closure, the linear scar can be very thin and easy to conceal with short haircuts.
The short-term cosmetic benefit with FUT is that the scalp is not shaved, so the patient looks completely the same one week after the surgery. We offer FUT Hair Transplants at our Los Angeles clinic. Will I have any scars?
Hair transplants may leave minor scars. FUE leaves tiny dots, while FUT leaves a thin linear scar, both easily hidden by hair. You can read more here.
Are there people who should not have hair transplants?
Hair transplants have a very high satisfaction rate when we choose the right patients. Here are a few examples of poor candidates:
It is a general rule that young patients (18-25 years old) are poor candidates. However, there are now medications to treat androgenic alopecia that we did not have several years back.
A patient who is unrealistic about what a hair transplant can accomplish. If a patient’s expectations do not match what I feel is realistic, then it is not a good idea to proceed with the surgery, and I will not perform it.
Finally, a patient who doesn’t want a hair transplant, but is getting the transplant because someone else wants the patient to.
As physicians, it is our responsibility to educate patients on all the treatment alternatives when they are not a good hair transplant candidate and to tell them why they should not have a hair transplant. Our number one responsibility is to be the patient’s advocate, even if it’s against their wishes.
How can I trust before and after photos?
I do show pictures to patients, but there is something you need to know about the photos:
For most of our patients, a hair transplant is a very private and personal matter; they do not want their pictures shown to other people. We put an extreme value on the confidentiality of our patients.
Pictures have been used in the past to deceive the patient. Different camera angles or lighting can make a person’s hair look like a “before and after” on the very same day. This is very misleading and gives the patients unrealistic expectations. We pride ourselves on matching expectations with reality.
Pictures generally give patients an appearance of having more hair.
If a potential patient wants to see our work, we have several patients who do not mind talking or meeting with them. When a candidate for hair transplants sees one of our patients in person, there is no way to deceive them; they see the results firsthand.
